Provider Transfer Service

When transitioning from a previous provider who is no longer available to continue your care, our Provider Transfer Service offers a seamless way to move forward. This service includes a thorough consultation, a detailed review of your previous care plan, and the development of a strategy to continue your care without interruption.

In some cases, additional information, such as lab work, may be required to ensure a comprehensive understanding of your medical history and eligibility for peptide therapy. If any necessary labs are identified during the consultation, we will coordinate these for you. Please note that the cost of lab work is separate from the base service fee.

Our goal is to ensure continuity and provide the highest quality care tailored to your individual needs. Let us help you take the next step with confidence and clarity.

**Copies of previous prescriptions or orders for peptides along with labs within the last 60 days should be emailed to info@exploringyourhealth.com after payment for this service has been made. If you don’t have these things available at your appointment, you will be converted to a peptide consultation and required lab work will be ordered (minimum $299).
